The relationship between the cervical spine, upper trapezius, and TMJ is one of the most underappreciated connections in musculoskeletal medicine. Understanding it can be the difference between managing symptoms forever and actually resolving them.
The Anatomy That Links Your Jaw to Your Neck
The temporomandibular joint sits just in front of each ear, connecting the mandible (lower jaw) to the temporal bone of the skull. What makes it uniquely vulnerable is its neurological neighborhood. The trigeminal nerve — the primary sensory nerve of the face and jaw — shares extensive connections with the upper cervical nerve roots (C1, C2, and C3) through a region called the trigeminocervical nucleus.

How the Upper Trapezius Fits In
The upper trapezius is one of the most chronically overloaded muscles in athletes who perform repetitive overhead, rotational, or sustained-posture activities — think golfers, tennis players, swimmers, and cyclists. When the upper trapezius is hypertonic (in a state of excessive tension), it pulls on the cervical spine and elevates the shoulder girdle, creating a chain reaction that ultimately reaches the jaw.

- Elevated shoulders shift the cervical spine into forward head posture.
- Forward head posture increases the load on upper cervical joints and shortens the suboccipital muscles.
- Shortened suboccipitals alter the resting jaw position, forcing the masticatory muscles to work harder to maintain occlusion (the contact between upper and lower teeth).
- Overworked masticatory muscles — particularly the masseter and temporalis — become hypertonic, compressing the TMJ and generating pain, clicking, and limited mouth opening.
For athletes, this cascade is often accelerated by sport-specific postures. A golfer spending hours in a rounded-spine address position, a tennis player perpetually reaching overhead, or a cyclist locked into an aggressive aero position all feed directly into the upper trap and cervical spine dysfunction that eventually shows up as jaw pain.
Why Standard TMJ Treatments Often Fall Short
The standard dental approach to TMJ dysfunction focuses on the joint itself: night guards, occlusal adjustments, and in severe cases, injections or surgery. These interventions can reduce local symptoms, but if the cervical and muscular drivers are never addressed, relief tends to be temporary.
Physical therapists who specialize in craniofacial and orthopedic conditions are trained to evaluate the whole picture. A comprehensive TMJ physical therapy assessment will typically include:
- Cervical spine mobility testing to identify restricted segments, particularly at C0-C1 and C1-C2
- Postural analysis to detect forward head posture, thoracic kyphosis, and shoulder elevation patterns
- Muscle palpation of the upper trapezius, sternocleidomastoid, suboccipitals, masseter, and temporalis

Athletes face a particular combination of variables that make this jaw-neck-shoulder triad more likely to become symptomatic:
High Training Volume and Muscle Fatigue
Chronic training fatigue — especially in season — reduces an athlete's ability to maintain ideal posture. As core and postural endurance drops, the upper trapezius and neck extensors compensate, building the tension that eventually reaches the jaw.
Stress and Clenching
Competitive athletes carry significant psychological stress. The masseter and temporalis are highly responsive to emotional load, and many athletes clench unconsciously during intense training, travel, or pre-competition anxiety. Combined with an already-strained cervical spine, this muscular bracing can rapidly escalate into a full TMJ flare.
Asymmetrical Movement Patterns
Sports like golf and tennis involve repeated rotation in one direction. Over time, these asymmetries create uneven loading in the cervical spine and upper trapezius, which can translate into asymmetrical jaw mechanics and unilateral TMJ symptoms.
Equipment and Ergonomics
Helmet fit, mouthguard design, and even the way an athlete holds a phone or reviews video footage between training sessions can influence cervical and jaw mechanics. These often-overlooked factors are worth evaluating when TMJ symptoms fail to respond to standard care.
What Effective Treatment Looks Like
The most effective approach to athlete TMJ dysfunction is integrative and collaborative. A physical therapist working in tandem with a dentist or oral medicine specialist can address both the joint mechanics and the upstream drivers simultaneously.
A well-designed TMJ physical therapy program for an athlete typically includes:
- Manual therapy to restore cervical mobility and reduce suboccipital tension
- Postural retraining targeting thoracic extension, scapular stability, and head-on-neck alignment
- Jaw-specific exercises to restore symmetrical opening, improve disc positioning, and build endurance in the deep jaw stabilizers
- Neuromuscular re-education to break the clenching habit and reduce resting muscle tone in the masseter and temporalis
- Sport-specific movement analysis to identify postures and patterns that need modification
- Load management guidance to support recovery during high training periods
The goal isn't just to eliminate jaw pain — it's to restore the entire jaw-neck-shoulder system to a state where it can handle athletic demand without recurring breakdown.
When to Seek Help

- Jaw pain or clicking that worsens after long training sessions

The jaw does not exist in isolation. It is the endpoint of a kinetic chain that begins in the thorax, travels through the cervical spine and upper trapezius, and terminates at the temporomandibular joint. Treat the whole chain, and the jaw finally gets a chance to heal.
